Mid-senior clinical sales liaison responsible for guiding patients from hospital to home-based hospice care by building referral networks, educating partners, and ensuring smooth care transitions while supporting organizational growth.

Key Responsibilities

  • Build and maintain referral relationships with hospitals, skilled nursing facilities, assisted living communities, physicians, case managers, discharge planners, and other community partners.
  • Educate healthcare professionals, patients, and families about hospice services, eligibility, benefits, and the admission process.
  • Identify appropriate hospice candidates and coordinate with the admissions team to facilitate smooth transitions of care.
  • Provide clinical guidance on hospice appropriateness, disease progression, symptom management, and goals-of-care discussions.
  • Conduct sales calls, presentations, educational in-services, networking events, and community outreach.
  • Collaborate with clinical and operational teams to support patient growth and quality care.
  • Monitor referral activity, market trends, and growth opportunities using CRM tools.
  • Ensure compliance with HIPAA, Medicare regulations, and ethical referral practices.
  • Achieve referral, relationship development, and territory growth goals while meeting monthly sales quotas.

Qualifications

  • Clinical background preferred (RN, LVN/LPN, Social Worker, or other healthcare experience).
  • Experience in hospice, home health, post-acute care, senior care, or healthcare sales is preferred.
  • Bachelor’s Degree in Business, Marketing, Healthcare, or related field.
  • Strong knowledge of hospice services and referral processes.
  • Excellent communication, presentation, and relationship-building skills.
  • Compassionate, organized, self-motivated, and comfortable discussing end-of-life care.
  • Valid driver’s license, reliable transportation, and local travel required.
  • Proficiency with CRM systems and Microsoft Office.
  • Established relationships with local hospitals, SNFs, ALFs, physicians, and case managers preferred.
  • Proven success growing referrals, admissions, or territory preferred.
  • Knowledge of Medicare hospice benefits and hospice levels of care preferred.
  • Experience delivering clinical presentations and educational programs preferred.
